We now have a dedicated social prescriber who holds a session each week at the surgery. The GP, practice nurse or community team can refer you (Although, to be honest all you need to do is ask one of us and we will refer you).
The social prescriber can link you into all of the amazing local voluntary groups and generally point you in the right direction for issues that you might be experiencing. If she does not know the answer she often knows someone who does.
