To help us assess your request quickly and safely, please keep it clear and concise.

The most helpful requests explain:

• what the problem is

• how long it has been going on

• what has changed or why you are contacting us now

• how it is affecting you

• what help you are hoping for

Our aim is to understand what you need as quickly as possible so we can get you to the right clinician, at the right time, in the right way.

We receive hundreds of requests each day. A clear, concise summary helps us decide who is best placed to help you, the most appropriate appointment type, and how urgently you need to be seen.

Artificial Intelligence (AI) can be a useful tool for helping you write your request, particularly if you find writing difficult. If you choose to use AI, please check that anything you submit is accurate, relevant and reflects your own circumstances. AI can sometimes include information that is inaccurate, irrelevant or unnecessarily detailed, which may delay our review or mean we need to contact you for clarification.

Please also be aware that some AI services may store information that you enter, so avoid sharing unnecessary personal information

Enhanced access

You may be offered an ‘improved access’ appointment, these doctor appointments are available across the South Cotswold locality every weekday evening until 8pm (except on bank holidays) and doctor and nurse appointments are available on a Saturday morning. If you are offered one of these appointments you might be seen in any one of the other South Cotswold surgery locations.

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Disabled access

There is easy wheelchair access to the surgery. Most consulting rooms are on the ground floor along with disabled toilet facilities. At our Avenue Surgery branch, some consulting rooms are on the 1st floor, which are accessible by a stair lift.

Invalid carriages can be parked by the main entrance and receptionists will always be ready to give any assistance required.

Parking

St Peter’s Road Surgery is less than two minutes from the central ‘Brewery car park’. There is also limited short term parking in St Peter’s Road.

The Avenue Surgery has limited parking on site, and is a four minute walk from ‘The Forum car park’.
