The practice along with the locality have concentrated on making sure that we identify patients with possible early dementia as there is lots that we can offer.
It was apparent a few years back that the number of patients that we had with dementia was not as many as one would expect. We have combined learning events, training and active case management to increase our identification rate and its now more in line with expected.
