Dr Gwynn is working with locality stakeholders to develop an integrated community service for patients with frailty.
There have been some successful stakeholder meetings to share the vision attended by Health Watch, Locality practices PPG’s, Cotswold District Council, voluntary sector and patient representatives. (Thanks to Nigel from our PPG for helping us put the meeting together).
Once we have shared the vision it will be onto the nitty gritty of the business case.
The end point will be a service that integrates primary, community and voluntary services around patients.
